Overview

The falls service is provided county-wide and provides home-based assessment to identify the factors and risks, which may contribute to a person’s falls.

The aim of the assessment is to develop a personalised action plan to enable the person to improve their confidence, balance, strength and stamina. In reducing the risks, we hope to boost independence and reduce rate and frequency of falls.

The assessment includes: mobility, self-care, bone strength, medications, home environment, medical factors, falls history, vision, hearing, foot health and memory/cognition.

If specific ongoing therapy is required, our teams can refer to physiotherapy or occupational therapy.

Referral: This service is provided for people who are 65 years and over who have had two fallls in the last six months or a fall that has resulted in a hospital admission or injury. Under 65's can be referred to therapy/nursing for general assessment.

Referral by GPs, other health professionals, voluntary services and by self-referral.

Home visits are provided by this Service.

Opening Hours:
 Monday - Friday  9am - 5pm.

Parking and Transport: Limited onsite parking. Accessible by public and community transport.

Access and Facilities: Main entrance well signposted and wheelchair accessible. Loop hearing aid system available. Wheelchair accessible toilet facilities.
