Amputee and Post Surgical Patients, Harnser Team, Colman Hospital

The service is provided for patients who would otherwise face an increased length of stay in an acute bed, which would delay the rehabilitation process and potentially increase the rehabilitation element of their care. Care would be assessed on the need and ability of each individual. This will include any assessments related to discharge to the most appropriate environment.

The amputee and post surgical unit comprises of an interdisciplinary team which includes doctors, nurses, physiotherapists and occupational therapists that have expert knowledge and training in assessment, treatment and management of the rehabilitation of amputee and post surgical patients. It has staff members whose sole responsibility is working within the specialism. Additional therapy following discharge from a hospital setting to patients requiring continued specialist amputee rehabilitation would continue to be provided by the amputee team in an outpatient capacity at the Colman Hospital.

The aim of the service is to maximise an individual’s potential following a surgical procedure enabling patients to continue living in their home environment. This would include people with other specialist needs (e.g. mental health and learning difficulties). Their rehabilitation needs would be met in the most appropriate environment to manage both physical and other specialist needs. Integrated health and social care staff using one set of notes and clear overall clinical and managerial leadership. The service provides a multidisciplinary team (MDT) approach with the appropriate skill mix to meet the needs of patients

Our services include:

In patients:

  • Patients who are unable to manage independently at home due to their limited function following an amputation where they have the potential to improve with the appropriate therapeutic input. They do not have the relevant family or social services support to enable them to function safely when left alone.

Out patients:

  • Patients who can return home with a level of independence and are considered to be safe would then be discharged from the ward environment and continue their rehabilitation as an outpatient within the ward therapy department. This could occur directly from acute care or Harnser Ward care following assessment. The aim of this service is to enable individuals to achieve their potential for managing an independent lifestyle, and take into account all the issues related to this on an individual basis.

Barley Court

  • This is a one bedroom unit in New Costessey which is part of a housing with care complex managed by a housing association within South Norfolk. The flat is available for an amputee patient who the Harnser team assess and consider would benefit from this setting when their home environment is considered unsuitable for their immediate return. Harnser Team remain responsible for the patients while they are in this facility jointly with Social Services. Patients who are considered suitable are those who would otherwise be able to return home with the relevant support however their current housing is unsuitable.

Who this Service is for

This service is available to all patients from Norfolk and would also include patients from areas whose needs could not be met within their own area. Patients from Ipswich, King’s Lynn and Great Yarmouth may be seen by a member of the Harnser Team in a satellite clinic or would be able to offer advice on the telephone. The team would also offer advice and support to other health care professionals within all the above areas, as well as the acute services within Norfolk.

Hours of Service

  • The inpatient amputee unit is operated on a 24/7 basis 365 days of the year and has different input from different members of the team depending on the day and time.
  • The Day Patient Service operates 08.30 – 16.30 Monday to Friday excluding bank holidays.
