Cardiac Rehabilitation Service
Cardiac rehabilitation is a process by which patients with cardiac disease, in partnership with a multidisciplinary team of health professionals, are encouraged and supported to achieve and maintain optimal physical and psychological health.
It is an individually designed programme relevant to the patient needs.
Cardiac rehabilitation programme aims:
- To help individuals with a heart condition to facilitate their recovery to return to normal living after a cardiac episode
- To improve patient's physical capacity
- To improve patient's ability to work and confidence to exercise safely
- To facilitate patient's psychological and emotional recovery
- To improve patient's perceived quality of life.
- To help patient's make necessary long term changes to their lifestyle to address adverse risk factors.
- To involve and support relevant carers
- To involve relevant community and voluntary organisations
- To increase uptake of the cardiac rehabilitation service
- To avoid hospital admission and readmission
- To avoid referrals to secondary care.
Who this Service is for
The service is indented for those patients registered with GP in Norfolk, excluding Great Yarmouth and Waveney area. Cardiac rehabilitation services for the West locality patients are offered by the Queen Elizabeth Hospital, Kings Lynn.
Patients are contacted by the service within 2 weeks after the receipt of referrals for arrangement of an appointment to carry out the initial assessment.
Suitable patients are invited to attend initial assessment and then the suitable patients are invited to attend the cardiac rehabilitation programme.
Access is also available to patients with MI, CABG, PCI, ACS, Valve repair, Heart failure, replacement valves and cardio-myopathy.
Following completion of cardiac rehabilitation, and post-rehabilitation assessment information on patients’ progress including results and future plans is sent to the patient, patient's GP or the practice nurse specialist or consultant if the refer is not the patients GP.
Inappropriate referrals are referred back to the referrer.
