Chronic Obstructive Pulmonary Disease (COPD) Service
The COPD service delivers specialist care to people in the community with a confirmed diagnosis of COPD.
The service aims to:
- To appropriately avoid admission for COPD patients in exacerbation or crisis.
- To facilitate discharges from the QEH to reduce the length of stay.
- To case manage and monitor high intensity users and liaise with mainstream community services.
- Provide expert opinion, advice and to other professionals to support the management of patients with COPD
- Improve local knowledge and provide education to other professionals to improve standards of care for patients with COPD
Who this Service is for
The service is accessible to all patients with a Norfolk GP (West Norfolk, Upwell, Fakenham and Wells). Patients in Norwich Community Hospital can access this service, as long as they have a West Norfolk GP.The team also support North Cambs Hospital. The service is also accessible to Lincolnshire and North Norfolk patients who have been admitted to QEH (or patients from any locality). These patients are not followed up in the community but are referred onto the relevant COPD service or Practice Nurse.
The service is accessed through a telephone/face to face or bleeped referral at the QEH. Any health professional may refer. For community referrals a form is completed again by any health professional. The service is also available for general advice. Patients who are already known to the team may self-refer.
