NHS Funded Continuing Healthcare

Anyone who is assessed as having a certain level of care needs may receive NHS Continuing Healthcare providing that the primary need is a health need. The first step for most individuals is the Healthcare Checklist Tool – a screening tool that is used by health and social care staff workers to identify that there is a possibility that an individual may be eligible for NHS Continuing Healthcare and a full assessment of eligibility is required. At this point a referral should be made to the Specialist Assessment Team (SAT).

SAT nurses co-ordinate assessment by Multidisciplinary team (MDT) professionals and only when these are completed will a recommendation be made that someone is eligible, or not, for NHS Funded Continuing Healthcare. The patient, and their carers, will always be involved in the assessment in a way that is appropriate to their circumstances.

At the end of the assessment the SAT nurse will facilitate a MDT meeting, along with the individual, and their carers, to make a recommendation of eligibility using the Department of Health’s National Framework Decision Support tool to decide whether the primary needs are health needs. This recommendation will then be ratified at a quality review Primary Care Trust panel, where the decision of eligibility for NHS Continuing Healthcare is made, and provided in writing.

NHS Continuing Healthcare is reassessed, and If health needs change then eligibility may also change.

 

NHS Funded Nursing Care

Assessment is required if you are resident within a care home that is registered to provide nursing, AND you do not qualify for NHS Continuing Healthcare but have been assessed as requiring the services of a Registered Nurse.

Reviews are completed by the SAT 3 months after admission, and at least annually. The patient and their carers will always be involved in the assessment in a way that is appropriate to their circumstances.

Sources of Referral

All professionals who have completed the healthcare checklist that indicates a full assessment for eligibility for NHS Continuing healthcare is required: Community Nurses, Specialist Nurses, Allied health professionals, Social services, Mental Health Trust professionals, GPs, Consultants, Care home staff (with or without nursing). Self referrals and carer requests (or inappropriate referrals to SAT) for both NHS Continuing healthcare checklists and Funded Nursing care determinations may be redirected to the most appropriate professional.
