Key Worker


The Norfolk Keyworker Service is provided for families of disabled children and young people often need to see a whole range of different health and care professionals, for a variety of reasons, at different stages and times of the child’s life.

Some families and carers can be overwhelmed by the many different organisations or people they may have to deal with; others may not even be aware of all of the services and support on offer to them.

Managed by experienced NHS staff who have worked within children’s services for many years, the main aim of the Norfolk Key Worker Service is to work closely with families / carers, to ensure that they can access all of the support and services out there, and that those services are working together to meet the needs of the child and family.

The service works closely with all of the organisations families may need to deal with, including children’s services, charities, and hospitals, providers of short break care, and even schools and GPs.

Who is the service for?
The Norfolk Key Worker Service is offered to disabled children and young people, from birth to age 19, who:

Are registered with a GP within greater Norfolk (excluding Great Yarmouth area)

Have complex health needs and / or a high level of need…
Complex health needs, can be severe health conditions which may affect a child / young person’s mobility, independence; and / or require regular and ongoing medical treatment. A child with a ‘high level of need’ is one who would be described as having a severe learning or physical disability which again may affect their mobility or independence.

Already see at least three different specialist health or care professionals from at least two different organisations …
These might be a therapist, social worker, or a home visitor from organisations such as Norfolk County Children’s Services (social or education services), NHS children’s services, housing services or voluntary groups.

The key worker will:

  • Be the main point of contact for the family and involved professionals across the agencies/organisations
  • Have regular contact with the family; listen to, support and work in partnership with the family
  • Be a source of information and advice, helping families/carers build their own confidence and be in control of the choices open to them
  • Meet regularly with the family and professionals to share information and agree a Family Service Plan

This service takes place within the community within schools and health centres. Home visits can also be provided by this service.

Opening Hours: Monday - Thursday 08:30 - 16:30  Home visits can be arranged out of hours if needed.

Parking and Transport: Limited onsite parking is available. Accessible by public and community transport.

Access and Facilities: Main entrance well signposted and wheelchair accessible. Wheelchair accessible toilet.

Referral Information: 19 and under. Referrals received from Health Professionals and a Carer with parental responsibility may self-refer.
