Urgent Community Response: Patient experience survey

Have you or somebody you care for recently received a 2-hour at home response from our Urgent Community Response team? We are inviting all patients who have received urgent care from us to share with us their experience. This will help us to thank your clinicians, develop your services and understand what is important to everyone we serve.

Each time you receive a Urgent Community Response from us, a clinician will request your feedback on every visit, every time either by paper copy (provided during the visit), link (as above) or through voluntary support over the phone. If you would like to receive voluntary support to provide feedback, please call our single point of contact team on 01603 518444. Out of hours (6pm to 8am) please call 01603 481234 and our staff will log the details of your request and contact the voluntary team to get in touch.
Each month your responses are collected and reviewed by our Quality teams, who then share good new stories with our teams and suggest service developments and improvements. We are incredibly grateful for your feedback – thank you!
