Our service

Providing rapid assessment to patients in west Norfolk who have been admitted to hospital but would prefer to remain within their own home.

The team, which includes an physiotherapist, occupational therapist, two specialist nurses, plus a technician, assess patients and organises appropriate support for them. This could see them return to their own home or another community setting.

To assist with safe and timely hospital discharge, patients are referred by emergency care and ambulance services for assessment and patients can request an additional assessment while the patient is in hospital.

Interpretation and translation services can be arranged by staff, for users of this service.
