System Update: E-Referrals to Community Services
Norfolk Community Health and Care NHS Trust (NCH&C) is moving all community nursing and therapy services onto a single SystmOne unit - NCH&C Norfolk Integrated Nursing and Therapy.
Currently teams work across multiple units. Moving to a single unit will:
· Improve patient care and continuity, with clinicians able to access full patient records instantly
· Enable closer collaboration between teams; reduce internal referrals and support multi-disciplinary working; streamline workflows through shared templates, protocols, and consistent safety alerts.
What this means for Primary Care
Whilst the referral process will remain unchanged, to enable the transition to one unit, referrers need to upload a new referral template. Unfortunately, this system update cannot be completed remotely by NCH&C.
To do the updates, GP practices need to upload the new referral template to the organisation group (or replace the local template where this is not in use). Process guides have been provided to support this update.
