Our service
The service provides short periods of 24-hour care to patients in their own homes to avoid unnecessary admissions to hospital and where appropriate to facilitate early discharge from hospital.
Interpretation and translation services can be arranged by staff, for users of this service.

Referrals
Referrals can be made by health and social care professionals. Please contact the Single Point of Contact - see details below.
Single Point of Contact
The Single Point of Contact administration office is open Monday to Sunday, 08:00-18:00, including bank holidays.
The team processes all referrals for community nursing and therapy, including routine and urgent care referrals for palliative and end-of-life care, as well as catheter and bowel care.

What to expect
When calling the service, you will initially be met with several options to ensure you are put through to the most suitable person.
We experience high volumes of calls between 08:00-10:30 and 14:00-15:30. If your call is not urgent, we advise calling outside these times.
When your call is answered, please have your GP surgery details and all relevant patient information ready.
Single Point of Contact is an administrative service and is unable to provide any clinical advice. Please be aware that the team is also unable to provide any information on when a nurse is due for a scheduled visit.
