This service provides assessment and support to help heart failure patients manage their long-term condition.
The service also provides monitoring during the treatment phase and follow-up thereafter by telephone or home visit.
Who we care for
Heart failure patients.
Referrals
Referral by health professional. Referrals are taken from primary and secondary care. Patients known to the service can self-refer following discharge from the service.
While you are waiting for an appointment
If you feel that your condition is worsening while you are waiting to be contacted, please speak to your GP to expedite your referral.
Help with health costs
If you're referred to hospital or other NHS premises for specialist NHS treatment or diagnostic tests by a doctor, dentist or another primary care health professional, you may be able to claim a refund of reasonable travel costs under the Healthcare Travel Costs Scheme (HTCS).
