Our service

The Community Neurology Team offers emotional and practical support and advice to people who have been diagnosed with neurological conditions, such as Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease or Huntington's Disease. We also support and offer advice to their families and friends.

The specialist multi-disciplinary team assess people's physical, emotional and psychological needs using functional and standardised assessments. This ensures adults with a neurological condition can live and work to their maximum ability.

Our teams include Specialist Neurology Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Clinical Psychologist and Highly Skilled Community Assistant Practitioners.

We offer home visits, provide care within clinics, carry out work-based assessments, and deliver gym therapy, aquatic therapy, or community access work. We can also help patients to return home from an acute hospital by liaising and signposting to other organisations if required. We also run Parkinson's Disease Education Groups.
