If you're referred to hospital or other NHS premises for specialist NHS treatment or diagnostic tests by a doctor, dentist or another primary care health professional, you may be able to claim a refund of reasonable travel costs under the Healthcare Travel Costs Scheme (HTCS). Our service

We are a multidisciplinary team, consisting of Nurses, Physiotherapists, Occupational Therapists and Assistant Practitioners based at the Norfolk and Norwich University Hospital.

We provide a 365-day service, serving the acute hospitals and GPs in Norfolk and Waveney. Our service has two aspects: Admission Avoidance (08:00 - 20:00) and Bed Based Community Hospital Unit Admissions (08:00 - 18:00). Our priority is helping avoid unnecessary admissions into acute hospitals.

Health professionals refer patients to us who are medically fit for transfer into a community unit or directly home with occupational therapy equipment and interim support. 

The CAT team assess the patient's needs and decide the appropriate service within NCH&C to support a safe transfer. Priority is given to palliative patients and admission avoidance patients. We liaise with our Community Inpatient Unit colleagues and fellow health and social care colleagues across the Norfolk and Waveney Integrated Care System.

 

Who we care for

Patients who attend the Norfolk & Norwich University Hospital (NNUH) A&E dept and assessment units and patients who are admitted to the NNUH wards who require rapid discharge support and/or rehabilitation.
