Our service

We support patients who need help managing ongoing treatment for chronic skin conditions such as psoriasis or eczema. A diagnosis should already have been made and a treatment plan prescribed. However, this can be adjusted to suit the patient's condition and needs where appropriate.

Home visits may be offered in specific circumstances.


Who we care for

We see both children and adults through the community dermatology nursing service. We work closely with health visitors, paediatricians and referrers to support ongoing care.


Referrals 

Any healthcare professional can refer to this service, including GPs, practice nurses, community nurses, pharmacists and secondary care clinicians.

Help with health costs

If you're referred to hospital or other NHS premises for specialist NHS treatment or diagnostic tests by a doctor, dentist or another primary care health professional, you may be able to claim a refund of reasonable travel costs under the Healthcare Travel Costs Scheme (HTCS).
