Help with health costs

If you're referred to hospital or other NHS premises for specialist NHS treatment or diagnostic tests by a doctor, dentist or another primary care health professional, you may be able to claim a refund of reasonable travel costs under the Healthcare Travel Costs Scheme (HTCS). Our service

This service helps patients over the age of 18, who have diabetes and may need specialist help with their diabetes. Patients can be seen at different places across West Norfolk or at home.

We offer diabetes education sessions for people newly diagnosed with type 2 diabetes are offered at different venues throughout West Norfolk. 

 

Who we care for

All patients over the age of 18 with diabetes, who need specialist care, can be seen in clinic, at home or at community clinic bases.

 

Referrals

All patients newly diagnosed with type 2 diabetes will be referred to this programme by their GP surgery. Also, any person living with type 2 diabetes and would like an update on diabetes can self-refer to attend these sessions by contacting the team via diabeteseducation@nchc.nhs.uk
